    1. I will take my temperature before leaving for my commitment, and I will not attend if I am running a fever, feeling ill, or have reason to suspect I’ve been exposed to COVID-19.
    2. I will wear a mask at all times when I am serving, and I will social distance between non-family members and myself.
    3. If I am diagnosed with COVID-19 within two weeks of my service, I will report this to the church so that others who attended the same service can be warned of possible exposure.
    4. I understand that any group activity poses a risk of contracting COVID-19 or another illness. In serving in this capacity, I assume that risk willingly and will not hold the church responsible if I become ill.
    5. I consent to photos and videos of myself (if over 18) and/or my minor-aged child’s activities while at the Event and understand these may be used in the promotion of BSUMC including web site entries, video blogs, or social media.
